- Insulation breakdown of the motor winding for phase U to the motor frame (ground), often due to moisture ingress, aging, or localized damage.
- Damage to the motor cable insulation for conductor U, creating a direct path to conduit, cable tray, or cabinet ground.
- Water ingress or condensation inside the motor terminal box, bridging phase U to the grounded enclosure.
- Improper or compromised grounding of the motor frame or cable shielding, leading to an unintended low-impedance ground path for phase U.
- Arcing, carbon tracking, or conductive debris from phase U to ground within the motor, drive output wiring compartment, or intermediate junction boxes.

1. Disconnect all motor power cables from drive output terminals U, V, W and disconnect motor frame ground (PE).

2. Perform an insulation resistance test (Megger) between motor terminal U and motor frame ground (PE) at 500VDC for 60 seconds (expected reading: >1 MOhm).

3. Inspect the motor cable for conductor U for physical damage to the outer jacket and internal insulation, especially at bends or points of stress to any grounded surface.

4. Check the motor terminal box for signs of moisture, conductive dust, loose wire strands, or corrosion bridging phase U to the grounded enclosure.

5. Verify the integrity of the motor frame ground connection, ensuring continuity to the protective earth (PE) busbar with an ohmmeter (expected <0.1 Ohm).

6. If motor and cable test good, measure insulation resistance between drive output terminal U and drive ground (PE) with a Megger (drive powered off, DC bus discharged) to check for an internal drive fault.
