• Improper wiring practices, such as running low-voltage safety I/O cables in close proximity to high-current power cables or unshielded motor leads.
• Internal hardware defect on the safety I/O module (Module 0 or Module 2) affecting test pulse generation or readback circuit integrity.
• Incorrect termination of cable shields at the panel ground bar, creating ground loops that interfere with signal integrity.
• Excessive electrical noise generated by nearby Variable Frequency Drives (VFDs), contactors, or switching power supplies impacting sensitive safety signals.
