- Accumulation of dirt, dust, or oil on the encoder's optical disk, interfering with track reading.
- Faulty encoder cable, causing intermittent loss of A, B, or Z phase signals during rotation.
- Internal processing error within the drive's encoder evaluation hardware causing incorrect track interpretation.
