Indicates a coin had been improperly routed within the changer. This can lead to coins ending up in the wrong tubes, jams, or incorrect inventory counts.

   Inspect all car position sensors (e.g., floor selector, encoder) for physical damage, debris, or misalignment that could lead to incorrect floor detection.

   Verify operational parameters within the Otis 400 controller relating to floor designations, car dispatch logic, and door timing against documented specifications.

   Check the integrity of wiring and connections for all hall calls, car calls, and floor position sensors for loose terminals or damage.
